Custom product intake forms
Complete the product-specific questions in your patient portal so your clinician has the information needed for a safe review.
Some medications need extra information that the standard intake doesn't cover — for example, a blood-pressure reading for BP-monitored weight-loss drugs.
When your clinician selects that kind of drug for your case, the system shows you a custom intake form on your dashboard that asks the questions specific to that drug.
What you'll see
The form looks like the lead-funnel quiz: one question per step, clear "Next" / "Back" buttons, and a final summary before you submit.
Question types you might see:
- Numeric input — e.g. "What's your latest systolic BP?"
- Single choice — e.g. "Do you currently smoke?"
- Multiple choice — e.g. "Which of these apply to you?"
- Free text — for context the clinician asked for
When do I see it?
Only when both are true:
- Your clinician has selected a product (drug) on your case
- That product has a custom intake form attached
If neither applies, no form appears — the standard intake is enough.
What does the system do with my answers?
Your answers are saved against your case and the clinician sees them on the case-detail screen. Two things happen automatically:
- Auto-recommendation — the system runs the product's triage rules over your answers and surfaces a recommendation (e.g. "approve" if BP < 140/90, or "review" / "flag" / "decline").
- The clinician still decides. The auto-recommendation is a signal, not a decision. The clinician reads it, looks at the underlying answers, and makes the call themselves.
This is by design. The platform never auto-approves or auto-declines a prescription. AI prepares the workflow; clinicians make the decision.
Can I edit my answers after submitting?
Not directly — but if you realise you made a mistake, message the clinic and they can re-open the form for you.
