Post-payment intake (BMI, ID, consent)

Submit your health information, BMI details, identity evidence, and consent so a UK clinic can review your request safely.

After payment, the clinician needs three things before they can issue a prescription:

  1. A BMI verification photo — proof your BMI matches what you entered on the funnel
  2. Photo ID — proof of identity (required for prescribing compliance)
  3. Signed consent form — UK GDPR + clinical consent

You'll find all three under the Outstanding requirements panel on /patient/dashboard.

BMI photo

Take a clear, well-lit photo of yourself on a digital scale that shows your current weight. Some clinics also ask for a body photo for completeness — follow the on-screen prompt.

Upload via the portal or WhatsApp the photo to the clinic's number — both routes attach to your case automatically. See the WhatsApp upload guide for detail.

Photo ID

Acceptable: passport, driving licence, or any photo ID with name + DOB visible. Cover address-only documents (utility bills etc.) are not sufficient. Upload one clear image.

Consent form

The consent form is in-portal — read each clause, tick to agree, and submit. The signed copy is stored on your record and in your Documents section for download. We snapshot the exact wording you signed (so any later wording change doesn't affect your record).

What if I don't complete these?

You'll receive reminders:

  • T+1 day — email reminder
  • T+3 days — WhatsApp reminder
  • T+7 days — final email reminder

Reminders stop automatically the moment you upload / sign. The clinic can't issue a prescription until all three are done.

What does the clinician do once I'm done?

The clinician verifies each item (BMI photo matches the BMI on file, ID matches your name, etc.) and either:

  • 🟢 Approves the prescription
  • 🟡 Requests more info (you'll see a message in the portal
    • WhatsApp)
  • 🔴 Declines with a reason
